(A) No person may drive a taxicab without having a valid Class C license and a current city-issued driver’s permit. A taxi operator may not employ any person as a driver unless the person has a valid Class C license.
(B) Original applications or renewals for a city driver’s permit shall contain:
(1) Applicant’s full name, current address, places of residence for the last five years and length of time of residence in the city;
(2) Applicant’s age, height and eye and hair color;
(3) Whether applicant has ever been convicted of a felony or misdemeanor that relates directly to applicant’s ability to perform duties as a driver and, if so, the date, time, place and nature or type of offenses;
(4) Whether applicant is able to speak, read and write the English language;
(5) Whether the applicant is free of defective vision, defective hearing, epilepsy, vertigo, heart trouble, communicable social diseases and any other infirmity, physical or mental, which would render him or her unfit for the safe operation of a taxicab;
(6) Applicant’s current Texas Class C driver’s license number and a copy of applicant’s moving violation record obtained from the Texas Department of Public Safety;
(7) The name of the operator for which applicant intends to work;
(8) Three recent passport-size photos of the applicant and a set of the applicant’s fingerprints, which fingerprints shall be taken by the Police Department of the city; and
(9) Other information as the city deems necessary to a fair determination of fitness as a driver.
(C) The application shall be verified under oath before a notary public.
