All applications shall be made at the office of the City Administrator upon forms prescribed by the proper agency of the state together with additional information as the Council may desire. A training verification form shall be filed with the application at the office of the City Administrator wherein the applicant verifies that all sellers or servers of liquor, beer or wine upon the licensed premises have successfully completed a responsible beverage service training course within the preceding 12 months and declare that all new sellers or servers will complete the training before beginning to sell or serve liquor, beer or wine. All questions or information required by the application forms shall be answered fully and completely by the applicant.
