713.04 REGISTRATION FOR CHARITABLE OR RELIGIOUS PURPOSE.
   (a)   Applications for registration certificates shall be filed with the Mayor or the Mayor's designee on a form to be furnished by the Mayor or the Mayor's designee. Each application shall contain:
      (1)   The name, address and the telephone number of the person completing the application;
      (2)    The name of an officer or official of the organization;
      (3)   The nature of the charitable or religious purpose to which the contributions, donations or sale proceeds will be applied; and
   (b)   An organization which desires to place a number of peddlers or solicitors in the Village simultaneously may make a group application to cover all of them; however, separate registration certificates shall be issued to each or, in lieu of separate registration certificates, separate information cards shall be issued to each peddler or solicitor by the organization. Such information cards shall include, at a minimum:
      (1)    The name of the organization;
      (2)   A description of the purpose of the peddling or solicitation;
                (3)    The period for which the registration certificate was issued;
                (4)    The name of the peddler or solicitor;
      (5)   A brief description of the peddler or solicitor, such as age, weight, and height; and
      (6)   The signatures of the peddler or solicitor and an officer or official of the organization.
   (c)   Any individual seeking a registration certificate, or any individual soliciting or peddling on behalf of an organization that has submitted a group application pursuant to subsection (b) above, shall be subject to a background check completed by any law enforcement authority.
   (d)   If the Mayor or the Mayor's designee determines after an investigation, that the information furnished under the requirements of subsection (a) above is correct; that the individual or individuals who will solicit or peddle on behalf of the organization listed in the registration application have not been convicted of a felony under Ohio, Federal, or any other state's laws; that said individual or individuals are not, at the time the application is made or anytime thereafter, on probation for any offense; that said individual or individuals are not, at the time the application is made or anytime thereafter, a parolee; that said individual or individuals are not, nor have ever been, a registered sex offender in Ohio or any other state he shall issue a registration certificate to the applicant.
   (e)   A registration certificate shall be valid for no more than a one-year period. The registration certificate shall state the expiration date.
   (f)   No fee shall be required for a registration certificate.
   (g)   Applications shall be made at least 48 hours before the registration certificate is issued and a registration certificate shall be issued no later than three business days after the submission of applications to the Mayor or the Mayor's designee
   (h)   The Mayor's or the Mayor's designee's denial of any registration certificate may be appealed pursuant to the process set forth in Section 713.05.
   (i)   No registration certificate shall be required for any person less than 16 years of age who acts as a solicitor or peddler on behalf of a religious or charitable organization.
   (j)   No registration certificate shall be required for any religious or charitable organization which employs the use of solicitors or peddlers under the age of 16, exclusively.
