§ 172.04 CONNECTIONS; PERMIT REQUIRED; BOND.
   (A)   No person shall uncover, make any connection with or opening into, use, alter or disturb any public sewer or appurtenance thereof without first obtaining a written permit from the village.
   (B)   The cost of the permit shall be $10 for each physical connection to the public sanitary sewer line.
   (C)   The property owner or his or her agent shall make application on a special permit form furnished by the village. The permit application shall be supplemented by any plans, specifications or other information considered pertinent in the judgment of the Superintendent.
   (D)   The Superintendent shall administer the collection of this fee, and he or she is hereby granted authority as may be necessary to collect the same, including the right to withhold issuance of a permit until plans are approved and until such fee is paid.
   (E)   The village may, at its discretion, require the person applying for such a permit to execute unto the village and deposit with the village a corporate surety in the sum of $1,000, conditioned that he or she will perform faithfully all work with due care and skill and in accordance with all laws, rules and regulations pertaining to plumbing. This bond shall state that the person will indemnify and save harmless the village and the owner of the premises against all damages, costs, expenses, outlays and claims of every kind arising out of unskillfulness or negligence on his or her part in connection with plumbing or excavating for plumbing as prescribed in this subchapter. Such bond shall remain in force and must be executed for a period of one year, except that on such expiration, it shall remain in force as to all penalties, claims and demands that may have accrued thereunder prior to such expiration.
   (F)   There shall be five classes of building sewer permits:
      (1)   Residential;
      (2)   Commercial;
      (3)   Governmental;
      (4)   Institutional; and
      (5)   Industrial.
   (G)   All costs and expenses incidental to the installation and connection of the building sewer shall be borne by the owner. The owner or the person installing the building sewer for said owner shall indemnify the village from any loss or damage that may directly or indirectly be occasioned by said installation.
