(A) It shall be unlawful for any person owning or operating a pocket billiard parlor in the city to allow any person under 21 years of age to loiter in the pocket billiard room or to play pocket billiards, unless accompanied by his parent or guardian, or with the written consent of his parent or guardian.
(B) Any parent or guardian willing to permit a minor over 14 years of age, who is then in his care, to enter a billiard room shall file personally with the Police Department of the city his written consent. The consent shall state the name, age, and address of the minor, the name and address of the person giving the consent, and the consent shall remain in full force and effect until revoked by the parent or guardian and the Police Department notified. A statement showing the relationship of the person giving consent shall be made if the person is not a parent or legal guardian. The parent or guardian shall also file a copy of the consent with the operator of the place of business showing thereon the date of filing of such consent with the Police Department.
(C) In the event the keeper of any billiard room is of the opinion that any person desiring admission thereto is under the age of 21 years, he shall require the person to certify his age in writing. No minor shall make a false certificate as to his age or use a forged permit from his parent or guardian.
