§ 113.02 APPLICATION PROCEDURE.
   Any person desiring to conduct or operate a restaurant shall make application to the Health Officer for a permit. The application shall be made on a printed form to be furnished by the Board of Health and shall set forth the name of the applicant and the location of the building to be used as a restaurant.
