717.03 PERMIT PROCEDURES.
   (a)   Permit applications may be picked up at the municipal offices and shall be filed with the Police Department on a form provided by the Village. The permit will be reviewed by the Chief of Police and the Zoning Administrator.
   (b)   Written permission by the property owner shall be submitted on a form provided by the Village.
   (c)   A valid inspection certificate from the Harrison County Health Department.
   (d)   A valid site plan shall be submitted with the application.
   (e)   Application information shall include information relative to all employees to be working in the food truck or at the location of the food truck and not be limited to:
      (1)   Name, date of birth, age, sex, address, telephone number, driver’s license.
      (2)   A valid photographic identification card.
      (3)   That the applicant has not been convicted of a felony or misdemeanor involving force, violence, fraud, theft, or a sexually-oriented offense.
