(a) An applicant for a pavement cut permit (street opening) shall pay a fee of one hundred dollars ($100.00) for an opening of one square yard in size in the paved portion of the street. For each additional square yard or part thereof, for openings made in the paved portion of a street, a fee commensurate with the replacement cost of the additional open cut will be added to the cost of the permitted opening at a ratio of one hundred dollars ($100.00) per square yard, fifty dollars ($50.00) per one-half square yard and twenty-five dollars ($25.00) per one-fourth square yard of cut.
Each permit for making an opening shall be confined to a single project and shall be issued by the Mayor or his designee. Application shall be made on a from provided by the Village, giving the exact location of the proposed opening, the kind of paving, the area and depth to be excavated, and such other facts as may known for the opening. The permit shall be issued only after a cash deposit sufficient to cover the cost of restoration has been posted with the Mayor and/or Clerk-Treasurer, conditioned upon prompt and satisfactory refilling of excavations ad restoration of all surfaces disturbed.
(b) The fee for openings in the treelawn area or sidewalk shall be fifty dollars ($50.00) per square yard or a fee commensurate with the excavation restoration cost of the opening areas.
(c) The Mayor or his/her designee is hereby authorized to waive permit fees on anode installations, pole settings or adjustments, fire hydrant installations or repairs, valve or curb box maintenance in tree-lawns and public works improvements by Village departments or their contractors, and, with the approval of the Village Engineer and/or the Street Department Superintendent, may make special arrangements in cases where it is desirable to install lines under sidewalks.
(d) Permit fees for the type of work and right of way location are hereby set forth, as follows:

(e) The permit fee is payable at the time the application is submitted.
(f) The material used in the repair of any municipal water and/or sanitary and/or storm water line shall be according to specifications set by the Street Superintendent. Materials used in the backfilling of any right-of-way opening shall be according to specifications set by the Street Superintendent. (Ord. 2014-34. Passed 9-18-14.)
