Application for a license for a mechanical musical device shall be made to the Fiscal Officer of the Village upon such forms as are prescribed therefor. The applicant shall state his or its name; whether an individual, partnership or corporation; if a partnership, the names and addresses of the partners composing the same; the addresses at which it is desired to maintain and operate the mechanical musical device; the nature of the business conducted, at that address; the make, name, model serial number and other identifying information with reference to the particular mechanical musical device which the applicant desires to be licensed; the name and address of the owner of such device and such further information as the Fiscal Officer, in his discretion may require.
