§ 113.09 BUSINESS LICENSE; INSURANCE AND INDEMNITY.
   (A)   Each applicant for a license shall provide a certificate of liability insurance to the City Planner upon a form approved by the City Attorney, insuring the applicant, and naming the City of Butler as co-insured, against the following liabilities and in the following amounts relative to such activity:
      (1)   Personal injury: $100,000 per occurrence and $300,000 in the aggregate;
      (2)   Property damage: $25,000 per occurrence and $50,000 in the aggregate; and
      (3)   State minimum, at least, for motor vehicle insurance coverage.
   (B)   Each applicant shall provide a document approved by the City Attorney, in which the applicant agrees to indemnify and hold harmless the City of Butler for losses or expenses arising out of the operation of his/her business.
   (C)   All business operations conducted under this chapter shall require the proof of worker’s compensation insurance on all employees or the submission of a State of Indiana affidavit of self-employment.
