§ 116.04 PERMIT APPLICATION.
   A permit application shall be prepared by the City Secretary and shall include the following information:
   (A)   Name;
   (B)   Address;
   (C)   Phone number;
   (D)   Driver's license number;
   (E)   Employer, if any;
   (F)   Suppliers;
   (G)   Commodity;
   (H)   Proof of liability insurance.
