It shall be the duty of every licensee to furnish to the
City Manager/Safety Director
, not later than the third day of each month, a full and complete statement of claims filed and judgments rendered against such licensee arising out of the operation of public hacks or vehicles, which statement shall be in such form as the
City Manager/Safety Director
shall prescribe. Failure to comply with the requirements of this section shall subject the offender to the penalty provided in Section 852.99. A separate offense shall be deemed committed each day during or on which a violation occurs or continues.
