§ 51.046 PERMIT APPLICATION.
   (A)   All users required to submit a wastewater discharge permit application shall provide the following information:
      (1)   Description of activities, facilities, and plant processes on the premises, including a list of all raw materials and chemicals used or stored at the facility which are, or could accidentally or intentionally be discharged to the sewage works;
      (2)   Number and type of employees, hours of operation, and proposed or actual hours of operation;
      (3)   Description of each product, the process or processes used in its manufacture, and the maximum and typical rates of production;
      (4)   Type and average and maximum amount of raw materials processed per day;
      (5)   Site plans, floor plans, mechanical and plumbing plans, and details to show all sewers, floor drains, and appurtenances by size, location, and elevation, and all points of discharge;
      (6)   The average and maximum daily flow projected to be discharged to the sewage works;
      (7)   Results of sampling and analysis of the wastewater proposed to be discharged to the sewage works, which identifies the nature and concentration, and mass, if requested, of pollutants specifically regulated under this chapter and any others known or suspected by the applicant to be present in the wastewater which could have a significant impact on the municipal wastewater treatment plant or receiving water quality. The samples shall be representative of daily operations and shall be analyzed according to applicable laboratory methods under 40 C.F.R. part 136 or Standard Methods if no analytical method is prescribed in 40 C.F.R. part 136. At minimum, instantaneous values shall be provided. The Superintendent may request additional data, including sufficient data to project representative daily maximum and monthly average values;
      (8)   Time and duration of discharges; and
      (9)   Any other information as may be deemed necessary by the Superintendent to evaluate the wastewater discharge permit application.
   (B)   Incomplete or inaccurate applications will not be processed and will be returned to the user for revision.
