§ 91.31 PERMIT APPLICATION.
   (A)   All applications for a permit required by this chapter shall be made on forms designated by the Clerk-Treasurer and shall include the following information:
      (1)   The full name and address of the business.
      (2)   The full name, business address and home address of the manager.
      (3)   A telephone number at which the Police Department can notify personnel of the business of a need for assistance at any time.
      (4)   The names, addresses and dates of birth of all alarm agents employed by the business.
   (B)   A business alarm business shall promptly notify the Clerk-Treasurer in writing of any change in the information contained in the registration form.
