The applicant for the license shall furnish to the Clerk/Treasurer a written application giving the proposed location of the theater, exhibition, circus or other public show, the written consent of the owner of the grounds, a statement of the number of employees, a description of the sanitary and toilet facilities of the grounds, and a sufficient description of the type of show or exhibition to apprise the Clerk/Treasurer of the nature of the same.
