§ 52.52  TERMINATION OF PERMIT.
   (A)   Any industrial user who violates the following conditions of its wastewater discharge permit or order or state or federal law, is subject to permit termination:
      (1)   Violation of permit conditions;
      (2)   Failure to accurately report the wastewater constituents and characteristics or discharges;
      (3)   Failure to accurately report significant changes in operation or wastewater constituents or characteristics;
      (4)   Refusal of reasonable access to the user’s premises for the purpose of inspection, monitoring or sampling;
      (5)   Falsifying self-monitoring reports;
      (6)   Tampering with monitoring equipment;
      (7)   Failure to meet effluent limitations;
      (8)   Failure to pay fines;
      (9)   Failure to pay sewer charges;
      (10)   Failure to meet compliance schedules; and
      (11)   Failure to complete a wastewater survey or the wastewater discharge permit application.
   (B)   Noncomplying industrial users shall be notified of the proposed termination of their non-domestic wastewater discharge permit and be offered an opportunity to show cause under § 52.50(C) why the proposed action should not be taken.
