§ 1028.09 PROOF OF INCOME FOR ELIGIBILITY; WHERE OBTAINED.

(A) All applicants for snow removal service under this chapter shall provide to the Department of Public Service proof of income for eligibility from the following services:

Proof of Income	Source
(1) Current H.E.A.P. letter	H.E.A.P. Office
(2) Assistance I.D.	County Employment & Family Services
(3) Federal Income Tax Form from the preceding year

(B) Other proof of income may also be considered.
