§ 832.03 APPLICATION FOR LICENSE; INFORMATION REQUIRED.
   (A)   Application for the license required by § 832.02 shall be made in writing to the Mayor or the Director of Public Safety, or in their absence, to the Chef of Police.
   (B)   The application shall set forth the name of the applicant and, if a corporation, the name and residence of the principal officers. It shall state the location for which the license is desired and shall contain a correct description of the land and premises together with the correct boundaries of the same and, if the land and premises contain more than one acre of ground, there shall be an accurate map thereof attached to the petition and identified with the name of the applicant. There shall be indicated on the application a description of all buildings and structures on the premises with a statement of the number and kind of rooms in the buildings and the purpose for which such buildings and structures are designed to be used in connection with the license.
   (C)   The application shall state the number of water closets or toilets on the premises and their location with respect to the premises.
