§ 816.04 LICENSE APPLICATION.
   (A)   The application for a juke box or mechanical amusement device license shall contain the following information:
      (1)   The name and address of the applicant and his or her age, date and place of birth;
      (2)   Prior felony convictions or convictions of any crimes involving moral turpitude of the applicant, if any;
      (3)   The place where the machine or device is to be displayed or operated and the business conducted at that place; and
      (4)   A description of the machine to be covered by the license, its mechanical features, the name of the manufacturer and the serial number.
   (B)   Such application shall be made out in duplicate. One copy shall be referred to the Police Chief and the other copy shall be referred to the Mayor.
