§ 110.04 APPLICATION.
   (A)   All applications required by this subchapter shall be made in writing on a form supplied by the Village Clerk.
   (B)   Said form shall state:
      (1)   The name of the applicant;
      (2)   The type of business or occupation in which the applicant intends to engage;
      (3)   The location of the building or establishment;
      (4)   The zoning district, if any;
      (5)   The anticipated business hours;
      (6)   The fee to be paid;
      (7)   The signature of the applicant; and
      (8)   Any such additional information as may be needed by the Village Clerk in the issuance of the license.
