§ 116.100 PHOTO IDENTIFICATION CARDS.
   (A)   The city shall furnish one photo identification card for each applicant and/or employee. A copy of the card shall be filed by the Department with the license application and the card shall be carried by the licensee.
   (B)   Each applicant and/or employee shall:
      (1)   Submit to the taking of two photographs by the Department;
      (2)   Sign his or her name to the identification cards; and
      (3)   Pay the cost of the photo identification card when issued, as defined in the currently adopted Government Records Access and Management Act, being U.C.A. Title 63G, Chapter 2, Part 1, or its successor statute, fee schedule.
