In addition to the grounds set forth in § 110.032 of this chapter, the following are grounds for a license enforcement action:
(A) Failure to comply with the terms of a permit issued by the city under Chapter 156 of this code of ordinances; and
(B) Three or more serious or grave disciplinary sanctions, as defined by the state, within a three- year period.
