§ 112.17 RECORDS, REPORTS AND FILINGS.
   (A)   All grantees shall keep complete and accurate books of account and records of its business and operations under and in connection with the terms of this chapter.
   (B)   The Director of Administration or his or her authorized agent shall have access to all such books of account and records of a grantee for the purpose of ascertaining the correctness of any and all reports and shall be given local access to such records upon reasonable request.
   (C)   In addition to the above, the grantee shall file annually with the Director of Administration, no later than 30 days after the end of the grantee’s fiscal year, an income statement applicable to its operations for the preceding 12-month period.
   (D)   Copies of all petitions, applications and communications submitted by the grantee to the Federal Communications Commission, Securities and Exchange Commission or any other federal or state regulatory commission or agency having jurisdiction in respect to any matters affecting cable television operations authorized pursuant to the franchise shall be available to the village upon request.
   (E)   All grantees shall maintain true and accurate maps of plats of all existing and proposed trunk and distribution lines. The maps will be kept on file and shall be made available to the Village Engineer upon request.
   (F)   Any false entry in the books of account or records of a grantee or false statements in the reports to the Director of Administration as to material fact, knowingly made by a grantee, shall constitute the breach of a material provision of this chapter and the franchise.
