The title sheet shall contain the name of the subdivision, street, road or court name, and the name of the village and county in which the subdivision is located. Scales, an index and a location map shall also be included. The title sheet shall also indicate the number of the sublots of the proposed subdivision. The name of the firm submitting the plans shall be shown in the lower right hand corner. Space shall be provided on the title sheet or the first sheet of the plans for approval of the proper authorities as follows:
