(A) The Plan Commission of the town shall prepare printed forms and require all applicants applying for the designation of real estate as an Urban Development Area to use such forms in making application for such designation.
(B) The Plan Commission shall consider the following items in preparing such printed forms:
(1) Proposed use of real estate;
(2) Proposed type and cost of buildings and equipment if improvements are proposed;
(3) Proposed operation and number of jobs to be created;
(4) Real estate description; and
(5) Proposed development of the real estate if improvements or buildings are not immediately contemplated.
