(A) Single-phase 120/240 volt, 100 ampere service.
(1) The utility will:
(a) Dig and backfill the trench for the cable;
(b) Provide the cable from the utility’s distribution lines to the meter on the structure. If the meter is not installed on the structure, the cable will be provided to the service entrance switch; and
(c) Provide all materials needed, except the following:
1. Conduit used on the structure;
2. Multi-position meter-mounting devices;
3. Conduit fittings used on the structure; and
4. Fusible or breaker disconnects.
(2) The charges for the above service will be as shown in Appendix B of this chapter.
(B) Single-phase 120/240 volt, 200 ampere service. The utility will provide the same services and materials as previously stated for 100 ampere service. The charges for 200 ampere electrical services installed underground shall be as shown in Appendix B of this chapter.
(C) Single-phase 120/240 volt, 400 ampere service. The utility will provide the same services and materials as previously stated for 100 and 200 ampere services. For single-phase service in excess of 200 amperes, the utility will supply the installation and trench from the pole or pad-mount transformer to the customer’s structure and the customer will supply the cable. The utility’s charge will be based on the actual cost of labor and equipment used to make the installation.
(D) Single-phase 120/240 volt, in excess of 400 amperes. For a customer desiring single-phase service in excess of 400 amperes, the utility will supply the installation and trench from the pole or pad-mount transformer to the customer’s structure and the customer will supply the cable and conduit. The utility’s charge will be based on the actual cost of labor and equipment used to make the installation.
(E) Three-phase service. The utility will provide the same services and materials as previously stated for 100 and 200 ampere single-phase services. For three-phase service, the customer will supply the installation and trench from the pole or pad-mount transformer to the customer’s structure and the customer will supply the cable and conduit. The utility’s charge will be based on the actual cost of labor and equipment used to make the installation.
