§ 52.033  EXTENSION PERMIT.
   (A)   Any party desiring to extend an existing water main shall first secure a permit for the purpose from the Water and Sewer Board. Application forms shall be secured through the township offices. No application shall be approved which does not comply with the laws of the state, the provisions of this chapter, and any rules and regulations of the state Department of Public Health.
   (B)   If the application for water use for extension of an existing water main is disapproved by the Water and Sewer Board, the applicant shall be provided with a copy of the application notifying the applicant of the reasons for disapproval.
   (C)   No water main extensions, or any other water systems, shall be installed to the township water supply without first securing approval of the Water and Sewer Board. No approval shall be granted by the Water and Sewer Board unless the applicant shall be in full compliance with all applicable state laws, as well as the Water Supply Cross Connection Rules of the state Department of Public Health, being R325 through R325.440 of the state Administrative Code. The applicant shall first contact and obtain the approval of the state Department of Public Health in writing before the Water and Sewer Board shall approve any cross connection with any public or private water supply other than that of the township.
   (D)   Applicant shall submit detailed engineer drawings of the proposed extension of the water main. The Water and Sewer Board shall have the right to specify the extent of the detail required on the engineering drawings. The Water and Sewer Board shall have the authority to establish terms of the extension permit, and applicant shall comply strictly with the terms in all particulars. Any installation of a water line extension not made in compliance with the terms of the permit shall be considered a violation of this chapter and punishable under the terms hereof.
