Application for a permit for the movement of a building or similar structure onto, along or over any public street in the Village shall be in writing, filed with the Commissioner of Buildings, and shall state:
(a) The location from which and to which the building or similar structure will be moved;
(b) The manner or method by which such movement is to be accomplished;
(c) A drawing or plat sufficiently detailed to show the proposed placement of the building or other structure on the location to which it is to be moved and showing that, when so moved and located, compliance will be had with the Land Planning and Zoning Code;
(d) The agreement of the applicant to pay any and all expense which the Village may incur as the result of the actual movement of such building or similar structure;
(e) An agreement to provide to the Village, as a part of the application, a policy of general liability insurance, insuring the Village against liability for personal injury, wrongful death and property damage, as hereinafter provided.
