§ 98.08 CONSTRUCTION PERMITS REQUIRED; FEES.
   (A)   A permit is required for construction or reconstruction work on any public walk, curb opening, or driveway. Said permit shall be obtained from the Public Works Director by the owner of the contiguous property or his or her agent. A licensed contractor must ascertain that a permit has been procured before he or she constructs or reconstructs any public walk, curb opening, or driveway.
   (B)   The permit must be kept on the site of the construction or reconstruction until completion of the work and be available to city representatives upon request.
   (C)   Application for a permit to construct or reconstruct a public walk, curb opening, or driveway shall be on forms furnished by the Public Works Director.
   (D)   (1)   Fees to be collected for the issuance of curb cuts, sidewalk, and driveway construction permits in conformity with the provisions of this section shall be paid by the applicant upon obtaining the aforesaid permits as set forth in § 35.70.
      (2)   Fees shall be payable at the office of the Public Works Director. In the case of reconstruction, fees may be waived at the discretion of the Director if it is determined that the condition giving rise to the need for reconstruction was caused by the city.
   (E)   An individual permit shall be required for each separate job.
