(A) Application for permits to connect with the sewerage system or to do plumbing work to be connected therewith must be made in writing to the city engineer by the owner of the property to be drained or his or her authorized agent.
(B) The application shall give the precise location of the property, the number of the lot, the name of the owner, and the name of the person employed to do the work, and shall be made on blanks furnished for that purpose. No permit shall be deemed to authorize anything not stated in the application. The city engineer, upon completion of the work, shall certify its acceptance on the permit form.
