§ 33.23 MEDICAL AND DENTAL INSURANCE.
   (A)   Each full-time non-seasonal, non- temporary city employee not covered by a collective bargaining agreement may elect to subscribe to and receive from the city medical insurance coverage for themselves and their eligible dependent family members. Effective December 1, 2017, employees who elect to participate in the city’s group medical plan (any plan tier: single; employee/child; or family) shall pay a monthly premium share via payroll deduction as follows:

   (B)   Effective July 1, 2006, part-time, non-temporary/ non-seasonal hourly city employees are eligible to participate in the city’s group medical insurance plan if they are hired to work a minimum of 1,250 hours per calendar year. Non-temporary, salaried/exempt part-time personnel will be eligible to participate in the city’s group medical insurance plan if they are hired based on their salary equating to at least 50% of the annual salary that a full-time salaried/exempt employee would receive who is similarly classified. Eligible part-time employees who elect participation in the city’s group medical insurance shall pay 50% of the city’s total monthly premium cost for medical insurance via payroll deduction. The city shall then pay the remaining 50% of the monthly medical insurance premium at the single, employee plus spouse, employee plus children, or family rates, depending upon the employee’s eligibility. Effective January 1, 2020, non- temporary/non-seasonal part-time hourly or salaried employees, who meet the work hours and/or salary eligibility criteria previously established, will be eligible to participate in the city’s group dental and vision plans at their own expense. They must pay 100% of the dental and vision premiums.
   (C)   (1)   Single dental. Each full- time non-seasonal/non-temporary employee not covered by a collective bargaining agreement may elect to subscribe to the city’s group dental insurance coverage for themselves. Effective December 1, 2017, non-temporary full-time employees who elect to participate in the city’s group dental plan (single tier only for themselves) shall pay a monthly premium share via payroll deduction as follows:

      (2)   Dependent dental. Eligible full-time employees may, at their own expense, elect to subscribe to group dental insurance coverage for dependent family members. The employee shall pay the additional cost for dependent coverage via payroll deduction. In order to participate in the dependent coverage, the employee must also take single dental coverage for themselves. The city shall not pay any portion of the monthly premiums for dependent dental coverage.
   (D)   Employee share(s) of group premium payments will be taken via payroll deduction from an employee’s bi-weekly pay, only if there are sufficient earnings in that pay period to cover the deduction. If there are insufficient earnings to cover the cost of the payroll deduction, then the employee will be responsible for paying for his or her share of the premium(s) via a method that is acceptable to the Finance Director.
   (E)   Effective January 1, 2020, the group insurance pool of which the city is a member is offering voluntary vision coverage. This coverage will be offered to eligible employees effective January 1, 2020, but all participating full-time and eligible non- temporary/non-seasonal part-time employees, who meet the work hours and/or salary eligibility criteria outlined in division (B) above, will be responsible for paying 100% of the premiums associated with participation.
