§ 52.071 PERMIT REVOCATION.
   (A)   If the governing state or federal regulations change, the town may revoke the permit granted. Additionally, any user who violated the following conditions in division (B) below of his or her permit or of this chapter, or of applicable state and federal regulations, is subject to having his or her permit revoked.
   (B)   Violations subjecting a user to possible revocation of his or her permit include, but are not limited to, the following:
      (1)   Failure of a user to accurately report the wastewater constituents and characteristics of its discharge;
      (2)   Failure of the user to report significant changes in operations or wastewater constituents and characteristics;
      (3)   Refusal of reasonable access to the user’s premises for the purpose of inspection or monitoring;
      (4)   Violation of conditions of the permit; and/or
      (5)   Falsifying reports or tampering with samples.
