(A) All applications for licenses authorized by this chapter shall be verified and shall be filed with the Clerk. The applications must state the applicant’s name in full and that he or she understands and has read and complied with the requirements and possesses the qualifications specified in the Liquor Control Act and this chapter. If the applicant is a copartnership, the names and addresses of all partners, and if a corporation, the names and addresses of all officers and directors, must be stated.
(B) Application must be subscribed by the applicant who shall state under oath and by notarized signature that the facts therein contained are true.
