§ 111.02 RECORDS.
   (A)   The licensee shall keep or cause to be kept a record in triplicate on a form numbered consecutively, written in ink without changes or erasing showing the exact time of acceptance of any article in the course of business, an accurate description of each and any article received so that same can be easily identified by giving number, color, size, unusual marks, the make and any other matter of identification, also the name and residence of the person from whom such article is received together with his or her height, weight, age, nationality, color, complexion, color of hair, eyes, sex and distinguishing marks, also the rate of interest charged, amount allowed on each article and all other terms of the transaction. A true copy of such record shall be open to the inspection of the Chief of Police or other member of the Police Department at all reasonable hours, and shall be kept for one year from true exact date of receipt.
   (B)   A true exact copy shall be delivered immediately to the person from whom such article was received and the original copy of such report shall be mailed or delivered to the Chief of Police before 12:00 noon on the first working day following the receipt of such article.
