(A) Written application for a license shall be made annually by the owner on a form provided by the town.
(B) The owner shall be identified by name, address and telephone number and the dog's or cat's sex, age, breed, color and call name and the rabies inoculation certificate number, if applicable, date and administering veterinarian given.
(C) The application shall take place at the office of the Clerk-Treasurer.
