§ 150.20 COUNTY’S INSTALLATION AND INSPECTION.
   (A)   The Boone County Highway Department shall make available to the applicant of a residential drive or an agricultural drive the manpower and equipment to install the driveway culvert, provide one load of cover aggregate and provide all necessary drainage work. The applicant is responsible for the purchase of the driveway culvert and end sections before any installation can be provided.
   (B)   The Boone County Highway Department will also provide the necessary drainage work around any mailbox approach if it is needed.
   (C)   The applicant of a multi-family and commercial or industrial drive shall be responsible for the purchase and installation of a driveway culvert, cover aggregate and all necessary drainage work according to the site plan and provisions of this subchapter.
   (D)   The Boone County Highway Department shall make an inspection of the premises to determine whether the driveway and mailbox approach as proposed conforms to the provisions of this subchapter. If the applicant is found to be in violation of this subchapter, he or she shall be notified in writing by the Boone County Highway Supervisor that the applicant is in violation and shall be given 30 days to conform to this subchapter. Continued violation of this subchapter shall result in the removal of existing approaches at the applicants expense by the Boone County Highway Department.
   (E)   The applicant would then have to refile for a new permit and pay the new permit fee.
