Each cannabis business shall complete a written application, provided by the City Clerk, which must contain the following.
(A) Applicant information.
(1) If the applicant is an individual: the name, date of birth, home address, telephone number, email address and a copy of a valid state identification of the person registering the business and anyone with a financial interest in the cannabis business;
(2) If the applicant is a partnership/noncorporate entity: the name, date of birth, home address, telephone number, email address and a copy of a valid state identification for each partner, principal, member thereof, and anyone with a financial interest in the cannabis business; and
(3) If the applicant is a corporation: the name, date of birth, home address, telephone number, email address, and a copy of a valid state identification of each principal officer, registered agent thereof, and anyone with a financial interest in the cannabis business, a copy of the corporation or LLC’s certificate of good standing issued by the Illinois Secretary of State.
(B) Legal name. The legal name of the cannabis business and any other names it may operate under;
(C) Location. The physical location, telephone number, and internet address of the cannabis business;
(D) Copy of documents. A copy of all documents submitted to the Illinois Department of Financial and Professional Regulation or the Illinois Department of Agriculture for issuance of a state license;
(E) License. A copy of the cannabis business’s state issued license and a statement as to whether the cannabis business is a qualified social equity applicant by the State of Illinois;
(F) Special use license. A copy of the special use license issued by the City Council;
(G) Site plan. A copy of the site plan as submitted to the Plan Commission;
(H) Hours. The hours of operation for the cannabis business;
(I) Agent. The name, address, date of birth and copy of state issued agent identification for the designated agent in charge and all other dispensing agents;
(J) Property owner. The property owner’s name, address, telephone number and email address and a valid copy of the lease agreement if applicable;
(K) Insurance. Proof of adequate insurance coverage for liability, worker’s compensation, tenant or owner’s insurance for the premises and equipment;
(L) Security plan.
(1) A copy of the cannabis business’s security plan that describes the cannabis business’s plans to:
(a) Prevent diversion, theft, or loss of cannabis and monetary funds;
(b) Monitor the activities in and around the cannabis business; and
(c) Restrict access to limited access areas and restricted access areas.
(2) The Chief of Police shall have the right to review the security plan.
(M) Application fee. Nonrefundable application fee of $2,000.
