Any person desiring a license under the provisions of this subchapter shall make an application in writing to the City Clerk stating the location and general character of the business to be conducted and the number of persons to be employed. The application shall identify the owner or owners, operator, superintendent and manager of the business. The application shall be accompanied by a copy of the articles of incorporation, in the event application is made by a corporation, and by a copy of the partnership agreement in effect at the time of application, if made by a partnership. The application shall further be accompanied by a certificate that compliance has been made with the Assumed Business Name Act, 805 ILCS 405/0.01 et seq., if applicable to the business operation of applicants.
