No person shall inter, cremate, deposit in a vault or otherwise dispose of any human body of any person having died within the corporate limits of the city, or which may have been found within the corporate limits of the same, until such person has received a permit so to do, as hereinafter provided, which permit shall bear date when issued, shall state the name of the deceased, the date and cause of death, the manner in which the body will be disposed of and the place of such disposal, the name of the person to whom the permit is issued and the name of the attending physician, midwife or coroner and shall be signed by the City Clerk.
