§ 14.39 LICENSE APPLICATION.
   (a)   Special event license application. An application for a special event license under this Division C shall request the following information:
      (1)   Whether the applicant is a natural person, corporation, partnership or other form of organization;
      (2)   The name and address of the person in charge of the special event;
      (3)   The name, address and date of birth of the person to be in charge of the and present at all times during the event;
      (4)   Whether the person in charge of the special event has ever used or been known by any other name, and if so, what was such name or names and information concerning dates and places where used;
      (5)   Whether the applicant has held more than two special event in the city in the previous 12 months;
      (6)   The specific area of the premises on which the event, including a floor plan that details the equipment to be operated, will be held and the date and time of the event;
      (7)   The specific type and number of equipment to be operated at the event;
      (8)   The number of people invited to attend the event;
      (9)   Whether any prizes are to be ultimately received by persons attending the event, and if so, the donors of the prizes; and
      (10)   Such other information as the City Council or may require.
   (b)   Annual establishment license application. The application for an annual establishment license under this Division C shall request the following information:
      (1)   The name of the establishment allowing to be conducted on its licensed premises;
      (2)   Whether the applicant is a natural person, corporation, partnership or other form of organization;
      (3)   Whether all real estate and personal property taxes that are due and payable for the licensed premises have been paid, and if not paid, the years and amounts that are unpaid;
      (4)   The type of license (on-sale intoxicating liquor, club, on-sale 3.2% malt liquor, (Type I or II) food or hotel/motel) the applicant currently holds; and
      (5)   Such other information as the City Council or may require.
