The system of administrative hearings for determination of eligibility for benefits under the public safety employee benefits act 1 shall be initiated by the serving of a written notice on the village administrator by any sworn police department employee stating the following in full detail:
A. The nature of the employee's claim giving full particulars thereof, i.e., date, time, place, nature of injury;
B. Any other factual circumstances surrounding said incident giving rise to said claim;
D. A request for the scheduling of a hearing for the eligibility to receive said benefits. (Ord. 2013-35, 11-25-2013)
