Any applicant for a license required by this chapter shall submit the following information.
(A) The full name and present address of the applicant or applicants.
(B) The two previous addresses immediately prior to the present address of the applicant and dates of residence.
(C) A description of service to be provided.
(D) The location and mailing address of the proposed establishment.
(E) If the applicant is a corporation, the names and residence addresses of each of the officers and directors of such corporation, and of each stockholder owning more than 10% of the corporation.
(F) If the applicant is a partnership, the names and residence addresses of each of the partners, including limited partners.
(G) Written proof, such as a birth certificate or sworn affidavit, that the applicant is at least 18 years of age.
(H) Individual applicant’s height, weight, sex, color of eyes and hair.
(I) One portrait photograph at least two inches by two inches and a complete set of the applicant’s fingerprints, which shall be taken by the Police Chief or his agent.
(J) Business, occupation or employment of the applicant for the three years immediately preceding the date of application.
(K) A certificate from a medical or osteopathic doctor stating that the applicant has, within 30 days immediately prior thereto, been examined and found to be free of any contagious or communicable disease.
(L) A statement as to whether or not such person, in previously operating in this or another city or state under license, has had such license revoked or suspended and the reason therefor, and the business activity or occupation subsequent to such action of suspension or revocation.
(M) All criminal convictions other than traffic violations and the reasons therefor.
(N) The application signed and sworn to by the applicant.
(O) If an application for a license to conduct a massage facility, the names and qualifications of all persons who are intended to give massages in the proposed facility.
(P) A statement allowing the Police Chief or his or her designee to conduct a criminal background check of the applicant, and the officers and directors of the applicant.
(Q) Such other identification and information necessary to discover the truth of the matters set forth in the application.
