(A) Application for the license required by § 114.02 shall be made to the City Clerk/Treasurer upon a form supplied for that purpose.
(B) The application for such license shall contain the following information:
(1) Name and address of applicant;
(2) Place where machine or device is to be displayed or operated and the business conducted at that place; and
(3) Any other information of a like nature required by the city.
