The owner will be sent written notice of the violation, which shall provide, at minimum:
(A) The name, address and owner of the real property where the violation has occurred;
(B) The type of violation that has occurred;
(C) The deadline for correcting the violation;
(D) The result of failing to meet the deadline; and
(E) The procedure for appealing the violation.
