(a) For the purpose of preventing backflow and providing cross-connection control in the public water system, Rules 3745-95-03 through 3745-95-08 of the Ohio Environmental Protection Agency Rules and Regulations for Public Water Systems, as were effective on November 26, 1980, and Rules 3745-95-01, 02, 04 through 07 of the Ohio Environmental Protection Agency Rules and Regulations for Public Water Systems, as were effective on May 1, 2003, are hereby adopted.
(1) "Director" as defined in Rule 3745-95-01(K) and "Supplier of Water" as defined in Rule 3745-95-01(AA) shall mean the Public Service Director.
(2) "Public Water System" as defined in Rule 3745-95-01(V) shall mean the City Public Water System.
(b) The Public Service Department is authorized to create Rules and Regulations on Cross-Connection Control based on the provisions of division (a) of this section, to include the following provisions:
(1) Specifications and requirements for cross-connection control devices.
(2) Requirement for annual inspections of backflow prevention devices by inspectors duly licensed by the Ohio Department of Commerce.
(3) Follow-up testing requirements in the event of unsatisfactory results from backflow prevention inspections.
(4) Provision to discontinue, after reasonable notice, water service to properties wherein any connection is in continued violation of the provisions of this chapter.
(5) Other lawful provisions necessary to effectively administer the requirements of this section.
(c) Water service that has been terminated for not conforming to the Backflow Prevention Device Testing Policy shall be assessed a service restoration charge as specified by Section 1042.03.
