4-7-302: PERMIT APPLICATION:
   A.   No person less than eighteen (18) years of age shall be eligible for an adult entertainer permit.
   B.   A completed application form signed by the applicant shall be submitted to the director at the time of application for an adult entertainer permit. The application form shall request the following information:
      1.   The applicant's legal name and any other names (including stage names and aliases) used by the applicant.
      2.   A state driver's license or state identification card. If the applicant does not possess a state driver's license or state identification card, then the applicant shall submit a description of the applicant that sets forth the applicant's date of birth, height, weight, and hair and eye color.
      3.   The applicant's present residence address.
      4.   Satisfactory written evidence that the applicant is at least eighteen (18) years of age.
      5.   Two (2) color photographs, taken within six (6) months prior to the date of application that clearly show the applicant's face.
      6.   A nonrefundable application fee in the amount of one hundred dollars ($100.00). This fee shall not be increased.
      7.   The business name and address where the applicant intends to perform.
   C.   If the director determines that the applicant has completed the application improperly, the director shall promptly notify the applicant of such fact and, upon request of the applicant, shall return the application. As a convenience to the applicant, upon request of the applicant, the director shall grant the applicant an extension of time of ten (10) calendar days to complete the application properly. The time period for granting or denying the requested permit shall be stayed during the period in which the applicant is granted an extension of time. Unless the applicant requests an extension of time pursuant to this subsection, nothing in this subsection shall be construed to relieve the director of the duty to make a decision within the time limits set forth in section 4-7-304 of this article.
   D.   Pursuant to California Government Code section 6254(f), information submitted in connection with an adult entertainer permit application is not a public record and shall not be disclosed by the city to any person other than city public safety personnel.
