(A) Upon completion of any flowing well, the wellhead equipment shall have, on the tubing, at least one master valve plus a flow valve, and a valve on the casing annulus.
(B) All wells shall be equipped with flow tubing of a size at least two inches inside diameter extending from not less than 50 feet from the top of the lowest producing formation.
