(A) Applications for an original franchise grant hereunder shall be filed with the City Clerk in accordance with the filing instructions promulgated by the city and will be accompanied by a non-refundable filing fee of $100.
(B) The applications shall contain written information, as outlined in the filing instructions and presented in the specific form required, which information shall include, but not be limited to the following:
(1) The name and business address of the applicant, date of application and signature of applicant or appropriate corporate officer(s);
(2) A general description of the applicant's proposed operations;
(C) A description of the services to be provided;
(D) A description of the educational and governmental access channels to be provided;
(E) Statement detailing and setting forth the names and addresses of all persons having proprietary or equitable interest in the franchise and in the event the applicant is a corporation, the names and addresses of the officers, directors and shareholders of said corporation together with the number of shares held by each shareholder, both nominal and beneficial, owning 10% or more of the outstanding shares of the corporation's outstanding stock;
(F) A statement describing all intracompany relationships of the applicant, if any, including parent, subsidiary or affiliated companies.
