The application for license shall be verified under oath by the applicant, shall be on forms prescribed by the city and shall include the following information:
(A) Name and address of the applicant;
(B) Location within the city for which the license is sought;
(C) Location(s) of other places where the applicant engages in new or used motor vehicle sales;
(D) Site plan showing the location and layout of the establishment, including lot width, lot depth, designated parking, any structure on the property and location of display area;
(E) Proof of license issued by the state; and
(F) Other information as the Community Development Department may require to evaluate the application.
