(A) No person, partnership, association corporation, or other entity shall establish, conduct or maintain in the city any nursing care facility or drug and/or alcohol abuse treatment facility or related facility without first obtaining a license and paying the tax required in § 110.02.
(B) Application for license shall be in writing, showing the location of the nursing care facility or drug and/or alcohol abuse treatment facility, showing the size and type of structure, the number of patients or persons to be housed therein, the name of the licensee, the number and date of the license issued by the State Department of Health or State Department of Mental Health Substance Abuse Services and the type of facility to be conducted.
